Please follow the procedures below
1.If you have a child who is 5 years old on or before December 1st, 2010 and request transportation to private school, you must register, in person, with the Lawrence School District Registration/Transportation Office. At that time, you must provide the following documents:
a. The child’s Original Birth Certificate
b. Proof of Residency (Deed, tax bill or closing statement)
If you rent, whether you have a lease or not, you will be given two affidavits that you must complete, sign and have notarized.
2. If you are a new resident in the district:
You must also register, in person, at the Registration/Transportation Office and have the following documents:
- Proof of Residency (Deed, Tax Bill or Closing Statement) If you rent, whether you have a lease or not, you will be given two affidavits that must be completed, signed and notarized.
- The Original Birth Certificate of the child/children that are to be registered.
3.Parents of children already registered can use the website to file a request for transportation and textbooks for the 2010-2011 School Year. The website will only be open until the deadline, April 1st. All other requests will be considered late and may be denied. Late requests for transportation are subject to availability. If for some reason, after you have filed a request form for transportation, you decide to change the school you originally requested, you must apply in person at the transportation office. No changes will be accepted over the website.
ALL WEB BASED APPLICATIONS THAT ARE SUBMITTED ARE CONSIDERED ORIGINALS. PLEASE RETAIN A COPY AS YOUR CONFIRMATION. IF THERE IS A CHANGE IN SCHOOLS, ADDRESSES, GRADES, ETC, AFTER FILING AN“ORIGINAL”, YOU MUST COME IN TO THE TRANSPORTATION OFFICE AND RE-SUBMIT A NEW FORM.

*MAXIMUM DISTANCE A CHILD WILL BE TRANSPORTED IS 15 MILES. ANY REQUEST OVER THE 15 MILE LIMIT, WILL BE DENIED.
